The following forms should be completed and uploaded into your online registration application:
- New Student Registration Information (en Español)
- New Student Registration Form (en Español)
- Home Language Questionnaire (en Español)
- Preferred Language Form (en Español)
- Census Information (en Español)
- Health Questionnaire (en Español)
- Health History Form (en Español)
- Health Examination Form (en Español)
In addition, you must provide one of the following as proof of residency:
If you own your home:
- A copy of the deed, mortgage statement, or tax bill.
- A copy of a utility bill, or any bill with homeowner's name and address
If you rent in a private home:
- Form A (en Español) and Form B (en Español) notarized
- Renter - Attaches a copy of the lease or rental agreement and a utility bill, or any bill with renter's name and address.
- Homeowner - Attaches a copy of their deed, mortgage statement, or tax bill.
If you rent in an apartment building/complex:
- Form A (en Español) and Form B2 (en Español) notarized
- Renter - Attaches a copy of the lease or rental agreement and a utility bill, or any bill with renter's name and address.
- Homeowner - Fills out Form B2. Needs form notarized.
